ACADEMIC OFFICER and/or designated representative(s) of the CARE Team. Describe as
completely as possible the behavior you observed, heard, or was reported to you. Use
direct quotes when reporting statements made by others. Also, please provide any documentation
you have (emails, notes, text messages, social media, etc.) which may help the CARE
Team determine the best method to address the situation.
